The first wave of glucagon-like peptide-1 (GLP-1) therapies applied beyond diabetes settled one question decisively: pharmacological intervention in obesity works. But the challenge has moved on. Pharma companies now face three critical tests: building evidence strategies that demonstrate measurable long-term outcomes as competition intensifies across assets and delivery models; making metabolic care work within healthcare systems that lack the pathways, payor frameworks, and primary care capacity to support it; and engaging patients and clinicians whose expectations are being shaped faster than any company can manage. This will take next-generation, cross-disciplinary business and scientific thinking. Yet many pharma companies are still operating with the last-generation playbook.
